Hospital Indemnity Insurance offers extra protection should you or a family member become hospitalized. Your current health insurance may not cover all of your hospital expenses, resulting in the rapid accumulation of your deductible, co-pays and other non-covered expenses. Once enrolled, the ACS Group Hospital Indemnity Plan pays you up to $300 a day - $2,100 a week - $8,400 a month – for up to 365 days, anytime you or a family member are hospitalized.* While hospitalization can be a difficult time, the plan can help you rest a little easier knowing those extra expenses may be paid, protecting your savings and retirement plans.

| Eligibility | Acceptance is GUARANTEED for all ACS members or Society Affiliates under age 65 who reside in the United States* (excluding territories) and Puerto Rico, regardless of health. Acceptance is also guaranteed for eligible spouses (under age 65) or domestic partner (under age 65) and dependent children under age 26 (including legally adopted children and step-children). Newborns are automatically covered for 31 days from birth. If the newborn is the first child, coverage can be extended by completing the appropriate enrollment forms and paying the additional premium. All future children are then automatically covered at no additional cost. *Certain state restrictions apply. |
| Choice of Benefits | You may select from six daily benefit amount options - $50, $100, $150, $200, $250, or $300 - depending on your needs. You can obtain coverage for yourself alone, or include your spouse/domestic partner and unmarried dependent children under age 26 (including legally adopted children and step-children.), or for your family. There is a maximum $50 daily benefit for children and the spousal daily benefit cannot exceed member daily benefit. |
| Program Highlights | Benefits are paid DIRECTLY to you. Benefits begin on the FIRST DAY of covered hospitalization. Benefits continue for up to 365 days for each period of confinement (maximum 20 days during any 12 month period in an insured's lifetime for confinement due to mental or nervous disorders). Daily benefits are DOUBLED for the first seven days of confinement in an intensive care or coronary care unit. Benefits equal to 50% of your selected benefit are payable for up to 180 days if you are confined to a convalescent facility after a hospital confinement of 15 days or more. | Coverage Effective Date | Your coverage will become effective on the day of approval of your application by New York Life Insurance Company, provided you pay your premium when due. If you, or any member of your family to be covered, are hospitalized on the date the insurance would become effective, coverage for that individual will not commence until the day after their discharge from the hospital. |
| Continuation of Coverage | Your insurance will continue automatically as long as you maintain your ACS membership or appropriate affiliation, you continue to pay your premiums when due, and the Group Policy remains in force. Your spouse and dependent children remain covered as long as they continue to meet the eligibility requirements, and you pay the appropriate premium. |
| Portable Protection | Coverage under the ACS Group Hospital Indemnity Insurance Plan is completely portable, staying with you even if you change jobs. Your cover continues as long as the group policy remains in effect, you maintain your ACS membership and pay your premiums when due. |

| Important Notice | How New York Life Underwrites Your Request for Group Hospital Indemnity Insurance Information regarding insurability will be treated as confidential. In considering your request for insurance, we will rely on the medical information you provide, and on the information you authorize us to obtain from your doctor, other medical practitioners and facilities, other insurance companies to which you have applied for insurance and MIB (Medical Information Bureau). New York Life will not disclose such information to anyone except those you authorize or where required or permitted by law. We may make a brief report to MIB; however, we will not disclose our underwriting decision. Information in our files may be seen by New York Life and Plan Administrator employees, but only on a “need to know” basis in considering your request. Upon receipt of all requested information, we will make a determination as to whether your request for insurance can be approved. MIB is a nonprofit organization of life insurance companies which operates an information exchange on behalf of its members. When you apply for insurance or submit a claim for benefits to a MIB member company, medical or non-medical information may be give to the Bureau, which may then be furnished to member companies. If we cannot provide the coverage you requested, we will tell you why. If you feel our information is inaccurate, you will be given a change to correct or complete the information in our files. Upon written request to New York Life or MIB, you will be provided with non-medical information. Generally, medical information will be given either directly to the proposed insured or to a medical professional designated by the proposed insured. Your request is handled in accordance with the Fair Credit Reporting Act procedures. If you question the accuracy of the information provided by MIB, you may contact MIB and seek a correction.
