Group 10-Year Level Term Life Insurance Plan

ACS Group 10-Year Level Term Life Insurance is an option designed to provide you with affordable fixed rates that are guaranteed not to increase with benefits that remain constant for the initial term period.

ACS Group 10-Year Level Term Life Insurance offers you quality coverage that helps you protect your family for the next decade. With your life insurance in place you can get back to focusing on life knowing that you have helped to protect your family financially should something happen to you.

Eligibility All ACS members and Society Affiliates who reside in the U.S.* (except territories) and Puerto Rico, are under age 65 may apply for 10 Year Level Term. Coverage is available to member’s spouses and domestic partners under age 65. All unmarried dependent children under age 26 are also eligible for coverage. *Certain state restrictions apply.
Coverage Options
  • Member options - $100,000 to $2,000,000 in $1,000 multiples
  • Spouse/Domestic Partner options - $100,000 to $2,000,000 in $1,000 multiples (not to exceed 100% member’s coverage).
  • Unmarried Dependent Child options – $500 under 6 months of age, $2,500 to $10,000 in $2,500 multiples for age 6 months through age 25.
The aggregate maximum amount of life insurance in force for all New York Life ACS group life insurance policies may not exceed $2,000,000 per insured individual.
Money Saving Discounts Affordable rates are possible through the power of group purchasing. Members may be eligible for savings such as: Volume discounts: Discounts for members who apply for higher amounts. If you or your spouse becomes insured for coverage amounts of $200,000 through $499,000 you’ll receive a volume discount. For $500,000 through $2,000,000 of coverage, you’ll receive an even bigger discount. Non-Smoker discounts: If you and/or your spouse have not used tobacco or nicotine products for the past 24 months, you may qualify for “Preferred” or “Select” rates that further reduce your premium.
Continued Interest Account This plan features a valuable Continued Interest Account, which means death benefits to a beneficiary, if the ACS member wishes, will be placed in an interest bearing account. Your beneficiaries can take their time evaluating their future financial plans, secure in the knowledge they may withdraw any or all of the death benefit proceeds at any time.
Accelerated Death Benefit If an insured member or spouse is diagnosed with a life expectancy of 12 months (Illinois residents: 24 months) or less, the insured may request one advance payment equal to 75% of his or her inforce life insurance to be paid while the terminally ill person is still alive. All money received under the Accelerated Death Benefit can be used to pay medical bills, take care of financial obligations or simply maintain ones quality of living. Please note that receipt of Accelerated Death Benefits may affect your eligibility for public assistance programs and may be taxable. Prior to applying to receive such benefits, you should consult with the appropriate social services and seek the advice of a qualified tax advisor. Note: The Accelerated Death Benefit is not available to residents of Massachusetts
Name Your Beneficiary Your beneficiary is the person (s) last designated by you or your spouse in writing and recorded by or on behalf of New York Life. You are the automatic beneficiary for dependent child insurance, as described in the certificate of insurance.
Exclusions Benefits are paid for death from any cause, at any time, anywhere in the world, except if relating to suicide within 24 months of coverage approval date. (Missouri residents: suicide is not excluded unless intended when you applied.) The validity of any amount of your life insurance, which has been in force for two years during an insured’s lifetime, will not be contested except for insurance eligibility provisions and non-payment of premium contributions.
Coverage Effective Date Coverage will take effect on the date the request for insurance is approved by New York Life, provided the person to be insured is performing the normal activities of a person in good health of like age on the date of approval (NC residents; perform the normal activities of a person of like age). The normal activities requirement does not apply to dependent life coverage for children.
When Coverage Ends Coverage will end when the insured member or spouse reaches 75 (26 for children) or earlier if: Premiums are not paid when due; The Group plan is terminated or modified by the Policyholder to end insurance for the group of insured to which the member belongs; If the insured requests to terminate the insurance; Dependent child coverage will terminate when the child is no longer an eligible dependent.
Coverage After the 10-Year Period Premiums are guaranteed to remain level for the first 10 years of coverage. At the end of the initial period, an insured and an insured’s spouse may reapply for a subsequent 10 year term of coverage at the rates then in effect, provided the insured person is under age 65. Rates for the subsequent term would be determined based on your then current age, health and tobacco/nicotine use status and be guaranteed for 10 years. If you or your spouse are not approved or do not apply for a subsequent 10 year term, coverage will continue in force on a non-guaranteed rate basis with increasing premiums as the insured ages. Portable Protection Coverage under the ACS Group 10-Year Level Term Life Plan is completely portable, staying with you even if you change jobs. Your coverage continues as long as the group policy remains in effect, you maintain your ACS membership and pay your premiums when due.
30-Day Free Look If you are not completely satisfied with the terms of your Certificate of Insurance, you may return it without claim, within 30 days. Your coverage will be invalidated and you will receive a full refund – no questions asked!
Rates Rates
Important Notice How New York Life Underwrites Your Request for Group 10 Year Level Term Life Insurance Information regarding insurability will be treated as confidential. In considering your request for insurance, we will rely on the medical information you provide, and on the information you authorize us to obtain from your doctor, other medical practitioners and facilities, other insurance companies to which you have applied for insurance and MIB (Medical Information Bureau). New York Life will not disclose such information to anyone except those you authorize or where required or permitted by law. We may make a brief report to MIB; however, we will not disclose our underwriting decision. Information in our files may be seen by New York Life and Plan Administrator employees, but only on a “need to know” basis in considering your request. Upon receipt of all requested information, we will make a determination as to whether your request for insurance can be approved. MIB is a nonprofit organization of life insurance companies which operates an information exchange on behalf of its members. When you apply for insurance or submit a claim for benefits to a MIB member company, medical or non-medical information may be give to the Bureau, which may then be furnished to member companies. If we cannot provide the coverage you requested, we will tell you why. If you feel our information is inaccurate, you will be given a change to correct or complete the information in our files. Upon written request to New York Life or MIB, you will be provided with non-medical information. Generally, medical information will be given either directly to the proposed insured or to a medical professional designated by the proposed insured. Your request is handled in accordance with the Fair Credit Reporting Act procedures. If you question the accuracy of the information provided by MIB, you may contact MIB and seek a correction. Please include your full name, date of birth, employer and address. Protected Person means a victim of domestic abuse: who has notified us that he/she is or has been a victim of domestic abuse; and who is an insured person or prospective insured person. Confidential Abuse Information means information about: acts of domestic abuse or abuse status; the work or home address or telephone number of a victim of domestic abuse; or the status of an applicant or insured as family member, employer or associate of a victim of domestic abuse or a person with whom an applicant or insured is known to have a direct, close personal, family or abuse related relationship. If we can provide the coverage you requested, we will inform you as to when such coverage will be effective. Under no circumstances will coverage be effective prior to this date.
