Group Term Life Insurance Plan

With coverage options of up to $2,000,000 and guaranteed renewability, ACS Group Term Life Insurance offers you quality coverage to help protect your future.

Eligibility All ACS members and Society Affiliates who reside in the U.S.* (except territories), Puerto Rico, or Canada (except Quebec) may apply for Basic Term Life ($25,000) protection for themselves and their spouse/domestic partner. Additional coverage – in $5,000 increments up to a maximum of $2,000,000 - is available to members and spouses/domestic partners under age 70. All unmarried dependent children under age 26 are also eligible for coverage. A dependent who is also a member is eligible for either member or dependent coverage but not both. If both spouse/domestic partners are covered as members, neither may insure the other as spouse/domestic partner and only one may insure any eligible children. * Certain state restrictions apply. Canada Residents: Pearl Insurance Group, LLC is acting solely as an administrator with respect to Canadian residents.
Coverage Options
  • Member options - $25,000 up to a maximum of $2,000,000. *
  • Spouse/Domestic Partner options - $25,000 up to $2,000,000 (not to exceed member’s coverage).*
  • Dependent Child – each unmarried, dependent child under age 26 (including legally adopted children) - $7,500 for children 1 and older; $400 for children under 1.
The aggregate maximum amount of life insurance in force for all New York Life ACS group life insurance policies may not exceed $2,000,000 per insured individual. *increased in increments of $5,000.
Inflation Guard This feature lets your coverage keep pace with the growing trends of inflation. You have the option to accept an automatic increase of 10% of your insurance amount for 10 years, gradually doubling the insurance amount. To receive each automatic increase, you must be under age 55 and not be totally disabled. Your total premium increases annually to reflect the increased coverage at your then current age. Once declined, this feature cannot be resumed.
Accidental Death & Dismemberment This policy automatically includes $15,000 of Accidental Death and Dismemberment coverage, which provides additional protection in the event of a fatal accident or covered injury such as loss of limbs or eyes – at no additional cost to you.
Disability Provision If you become totally and permanently disabled prior to age 60, you will be eligible to collect the first $25,000 of Basic Term Life insurance over a five year period. (See Certificate of Insurance for details). The application must be made no later than one year from the date you became disabled. The premium on any additional coverage on the member, as well as any spousal/domestic partner or dependent child coverage, will be waived while the total disability continues.
Continuation of Coverage Your insurance will continue automatically as long as you maintain your ACS membership you continue to pay your premiums when due, and the Group Policy remains in force. Your spouse and dependent children remain covered as long as they continue to meet the eligibility requirements, and you pay the appropriate premium.
Accelerated Death Benefit If an insured member or spouse/domestic partner is diagnosed as terminally ill with a life expectancy of 12 months (Illinois residents; 24 months) or less – the insured may request one advance payment equal to 75% of his or her in force life insurance while they are alive. Full premiums continue to be payable. For terms, conditions, and limitations, please see Certificate of Insurance. Please note that receipt of Accelerated Death Benefits may affect your eligibility for public assistance programs and may be taxable. Prior to applying to receive such benefits, you should consult with the appropriate social services and seek the advice of a qualified tax advisor. Note: The Accelerated Death Benefit is not available to residents of Massachusetts.
Coverage Effective Date Coverage is effective on the day of approval of your application, provided you pay your premium when due and are performing normal activities of a person in good health of like age on that day. (NC residents: performing normal activities of like age). If you are temporarily disabled at the time your application is approved, coverage will be effective on the first day you return to normal activities. Coverage for dependents will be effective on the same day as your coverage, provided they are not totally disabled on that day. Additional coverage for you or a dependent spouse/domestic partner will become effective on the day of approval of the application by the insurance company. (New York Life Insurance Company). All applications for coverage are contingent upon the insurance company’s (New York Life Insurance Company) approval of satisfactory evidence of insurability.
Converting a Term Life Policy If you choose to discontinue your ACS membership, you can convert all of part of your coverage to an individual permanent life policy, regardless of your physical condition, provided that you apply within 31 days after the date your insurance under your policy terminates. Your new premium will be based on your current age. Your dependents may also convert their coverage if you discontinue your membership with the ACS or when they cease to be eligible as dependents.
Premium Credit The Board of Trustees, Group Insurance Plans for ACS Members regularly approves the distribution of premium credits on new and renewed policies. The premium credit for the year 2009 is 20%. These premium credits are paid to the insurance carrier by the Life Plan Trust Fund. Premium credits are not guaranteed and vary from year-to-year depending on claims experience.
Portable Protection Coverage under the ACS Group Term Life Insurance Plan is completely portable, staying with you even if you change jobs. Your coverage continues as long as the group policy remains in effect, you maintain your ACS membership and pay your premiums when due.
30- Day Free Look If you are not completely satisfied with the terms of your Certificate of Insurance, you may return it without claim, within 30 days. Your coverage will be invalidated and you will receive a full refund – no questions asked!

Important Notice How New York Life Underwrites Your Request for Group Term Life Insurance Information regarding insurability will be treated as confidential. In considering your request for insurance, we will rely on the medical information you provide, and on the information you authorize us to obtain from your doctor, other medical practitioners and facilities, other insurance companies to which you have applied for insurance and MIB (Medical Information Bureau). New York Life will not disclose such information to anyone except those you authorize or where required or permitted by law. We may make a brief report to MIB; however, we will not disclose our underwriting decision. Information in our files may be seen by New York Life and Plan Administrator employees, but only on a “need to know” basis in considering your request. Upon receipt of all requested information, we will make a determination as to whether your request for insurance can be approved. MIB is a nonprofit organization of life insurance companies which operates an information exchange on behalf of its members. When you apply for insurance or submit a claim for benefits to a MIB member company, medical or non-medical information may be given to the Bureau, which may then be furnished to member companies. If we cannot provide the coverage you requested, we will tell you why. If you feel our information is inaccurate, you will be given a change to correct or complete the information in our files. Upon written request to New York Life or MIB, you will be provided with non-medical information. Generally, medical information will be given either directly to the proposed insured or to a medical professional designated by the proposed insured. Your request is handled in accordance with the Fair Credit Reporting Act procedures. If you question the accuracy of the information provided by MIB, you may contact MIB and seek a correction. For NM Residents: Protected Persons have a right of access to certain Confidential Abuse Information we maintain in our files and they may choose to receive such information directly. Please include your full name, date of birth, employer and address. Protected Person means a victim of domestic abuse: who has notified us that he/she is or has been a victim of domestic abuse; and who is an insured person or prospective insured person. Confidential Abuse Information means information about: acts of domestic abuse or abuse status; the work or home address or telephone number of a victim of domestic abuse; or the status of an applicant or insured as family member, employer or associate of a victim of domestic abuse or a person with whom an applicant or insured is known to have a direct, close, personal, family or abuse- related relationship. If we can provide the coverage you requested, we will inform you as to when such coverage will be effective. Under no circumstances will coverage be effective prior to this date.
